Abstract
This is a case report on a patient presenting with radicular symptoms caused by ganglion cyst mimicking down-migrated disc herniation. A 38-year-old male presented with a 2-month duration of low back pain followed by severe pain down the left leg. His sensation was reduced along the lateral thigh of the left leg. Spinal magnetic resonance imaging(MRI) demonstrated a compressing lesion to the dural sac at the L3-L4 level. Axial MRI images revealed iso-signal intensity on T1-weighted image and high-signal intensity on T2-weighted image. The patient underwent partial hemilaminectomy at L3-4 and L4 traversing nerve root was compressed and displaced medially by a well-encapsulated cyst. The walls of the cyst were dissected from the dura. The cyst was found to arise from the annulus fibrosus and completely removed. Pathologic diagnosis was ganglion cyst. Surgical removal is treatment of choice in patients with intraspinal ganglion cyst. Postoperatively, the patient was neurologically normal without any deficits.
