Objective
No studies assessing the prevalence of ossification of the spinal ligament were conducted using whole-spine positron emission tomography and computed tomography (PET-CT) in healthy Koreans. We aimed to determine the prevalence of ossification of the posterior longitudinal ligament (OPLL) and ossification of the ligament flavum (OLF) in healthy Koreans using whole-body PET-CT.
Methods
We reviewed whole-body PET-CT images captured during general health checkups at the General Health Promotion Center of our institution from January 2015 to 2020. OPLL and OLF were identified by the presence of heterotopic ossification in the posterior longitudinal ligament and ligament flavum on axial and sagittal PET-CT images.
Results
A total of 1,934 adults (1,645 men, 289 women) were included. The mean age was 48.05 years (range, 28–86 years). Among the 1,934 patients, 173 had OPLL (8.9%). The most commonly involved cervical vertebra levels arranged according to frequency were C4, C5, C3, and C6. OLF was observed in 125 patients (6.5%). The most commonly involved thoracic levels were T10, T11, and T5. The prevalence of OPLL and OLF was the highest in patients aged 60–69 years. Among the C-OPLL patients, 15.1% had T-OPLL, 5.0% had L-OPLL, and 25.8% had T-OLF.
Conclusion
Our study revealed the prevalence of OPLL and OLF in healthy Korean subjects. It was consistent with that in other Asian countries. The presence of OPLL and OLF at most locations correlated with the presence or absence of spinal ossification at other locations.

Chronic Opioid Use Following Lumbar Discectomy: Prevalence, Risk Factors, and Current Trends in the United States

Objective
Lumbar discectomy is commonly performed for symptomatic lumbar disc herniation. We aimed to examine prescribing patterns and risk factors for chronic opioid use following lumbar discectomy.
Methods
Using a private insurance claims database, patients were identified who underwent primary lumbar discectomy from 2010–2015 and had 1-year of continuous enrollment postoperatively. Patients were excluded with spinal fusion. The strength of opioid prescriptions was quantified using morphine milligram equivalents daily (MMED). Univariate and multivariate logistic regression models were built to examine risk factors associated with chronic postoperative opioid use.
Results
A total of 5,315 patients were included in the study (mean age, 59 years; 50% female). 1,198 of patients (23%) used chronic opioids postoperatively. Chronic opioid use declined significantly from 27% in 2010 to 17% in 2015, p < 0.001. In addition, there were significantly fewer patients receiving high and very high-dose opioid prescriptions from 2010–2015, p < 0.001. The median duration that patients used opioids postoperatively was 211 days in 2010 (interquartile range [IQR], 29–356 days), and decreased significantly to 44 days (IQR, 10–294 days) in 2015. The strongest factors associated with chronic opioid use were preoperative opioid use (odds ratio [OR], 4.0), drug abuse (OR, 2.6), depression (OR, 1.6), surgery in the west (OR, 1.6) or south (OR, 1.6), anxiety (OR, 1.5), or 30-day readmission (OR, 1.4).
Conclusion
Chronic opioid use following primary lumbar discectomy has declined from 2010–2015. A variety of factors are associated with chronic opioid use. Preoperative recognition of some of these risk factors may aid in perioperative management and counseling.

Ossification of the posterior longitudinal ligament (OPLL) in the cervical spine and related neurological complications are not uncommon in East Asian countries. The estimated prevalence of cervical OPLL-related hospitalization is 7.7 per 100,000 person-years in Taiwan, and higher incidence rates have been observed in elderly and male patients. Although cervical OPLL is frequently insidious, it can eventually cause myelopathy and predispose patients to spinal cord injury (SCI). There are multiple options for managing cervical OPLL, ranging from observation to many kinds of surgical procedures, including posterior laminoplasty, laminectomy with or without fusion, anterior corpectomy with or without instrumentation, and circumferential decompression and fusion. None of these surgical approaches is free of complications. However, to date, there is still a lack of consensus regarding the choice of the surgical approach and the timing of surgical intervention. Cervical SCI and related neurological disabilities are more likely to occur in OPLL patients, who should therefore be cautioned regarding the possibility of a subsequent SCI if treated without surgery. This article aimed to review the prevalence, management strategies, and prognosis of cervical OPLL.

Objective

The authors surveyed the prevalence and the clinical character of lumbar disc herniation (LDH) in Korean male adolescents, and the usefulness of current conscription criteria.

Methods

The data of 39,673 nineteen-year-old males that underwent a conscription examination at the Seoul Regional Korean Military Manpower Administration (MMA) from October 2010 to May 2011 were investigated. For those diagnosed as having lumbar disc herniation, prevalences, subject characteristics, herniation severities, levels of herniation, and modified Korean Oswestry low back pain disability scores by MMA physical grade were evaluated. The analysis was performed using medical certificates, medical records, medical images, and electromyographic and radiologic findings.

Results

The prevalence of adolescent LDH was 0.60%(237 of the 39,673 study subjects), and the prevalence of serious adolescent LDH with thecal sac compression or significant discogenic spinal stenosis was 0.28%(110 of the 39,673 study subjects). Of the 237 adolescent LDH cases, 105 (44.3%) were of single level LDH and 132 (55.7%) were of multiple level LDH, and the L4-5 level was the most severely and frequently affected. Oswestry back pain disability scores increased with herniation severity (p<0.01), and were well correlated with MMA grade.

Conclusions

In this large cohort of 19-year-old Korean males, the prevalence of adolescent LDH was 0.60% and the prevalence of serious adolescent LDH, which requires management, was relatively high at 0.28%. MMA physical grade was confirmed to be a useful measure of the disability caused by LDH.

OBJECTIVE
We applied chest radiographs to scoliosis screening for conscription. Prevalence, types of scoliosis, and insight of examinees with scoliosis were investigated.
METHODS
In this study, chest radiographs of 2417 males, who had been given an examination for conscription at the Seoul Regional Military Manpower Administration from April 2009 to May 2009, were analyzed. The prevalence of scoliosis more than a 10 degrees Cobb angle was calculated. The insight of scoliosis was investigated in every examinee and thoracolumbar radiographs were checked in those examinees with more than a 20 degree Cobb angle.
RESULTS
Among 1904 males, 477 (19.7%) exhibited scoliosis involving more than a 5 degrees Cobb angle were and 131 (5.4%) exhibitedmore than a 10 degree Cobb angle. In those 131 cases, 18 (13.7%) had a known history of problems with scoliosis. Among the group measuring less than a 10 degree Cobb angle, 1.7% of them misunderstood scoliosis. Insight of scoliosis increased according to the severity of spinal curvature; however, nearly half of the cases with a 20 degree or greater Cobb angle had no insight with respect to their scoliosis.
CONCLUSION
In male adolescents, the prevalence of scoliosis with a greater than 10 degree Cobb angle was 5.4% and there was a low insight with respect to scoliosis.

OBJECTIVE
In Korea, there appears to be an increase in the diagnosis of lumbar spine disorders recently. This trend has urged employers to screen new employees for this disorder. One of the methods for the investigation is the radiological examination of the lumbar spine. We studied the prevalence of spondylolysis according to age and sex with lumbar X-ray (anterior-posterior view and lateral view) of new employees in pre-employment screening examination. We also estimated the outbreak period of spondylolysis by analysing the prevalence rates at each age group, which were the results of our study.
METHODS
We gathered the data of lumbar X-ray findings for new employees, who had recieved pre-employment screening examinations in the department of occupational and environmental medicine of Masan Samsung Hospital Korea during a period from January 1, 2002 to December 31, 2005. We analyzed the data to obtain the age and sex prevalence of spondylolysis and spina bifida.
RESULTS
The prevalence of spondylolysis for the pre-employment screening examination was 5.9% (male 6.6%; female 4.3%) in this study. The prevalence of spondylolysis after adjusting age and sex was 9.0%. The prevalence of spondylolysis was positively correlated with age, and showed to be higher in female than male. Body mass index of spondylolytic subjects was higher than that of normal. The prevalence of spondylolysis was significantly higher in those with spina bifida than normal subjects.
CONCLUSION
The prevalence of spondylolysis of the pre-employment screening examination was 5.9% in this study. The factors which relates with spondylolysis were age, BMI and the prevalence of spina bifida.

OBJECTIVE
To evaluate the prevalence and the epidemiological findings of scoliosis in 19-year-old Korean males.
METHODS
We applied chest radiographs for scoliosis screening from the examinations for conscription at Regional Military Manpower Administration in Seoul, Korea, from April 2009 to December 2009. A total 28,779 males, aged 19 years, were screened for scoliosis. Additional thoraco-lumbar radiographs were obtained for every examinee identified with scoliosis of more than 20 degrees using Cobb's angle.
RESULTS
The prevalence of scoliosis of more than 10 degrees of Cobb's angle was 5.3%(1532 of 28,779 males). The most common category for these 19-year-old males was scoliosis of 10 to 19 degrees. The most common pattern of scoliosis was thoracic (57.6%), followed by thoracolumbar (32.1%), lumbar (7.0%), and double major curves (3.3%) scoliosis.
CONCLUSIONS
Among 28,779 19-year-old Korean males, the prevalence of scoliosis of more than 10 degrees Cobb angle was 5.3%, and thoracic scoliosis was the most common.
