Lumbar intraspinal synovial cysts are included in the difference diagnosis of lumbar radiculopathy. Developing imaging modalities has result in increased reporting about these lesions. However, the case of bilateral new lumbar intraspinal synovial cysts after laminectomy has been rarely reported. We report of a rare case with bilateral lumbar intraspinal synovial cysts after laminectomy, requiring surgical excision.

Spinal cord compressing syndrome due to synovial cyst (SC) of the thoracolumbar spine is a rare clinical condition. In this report we aim to heighten awareness of the thoracolumbar facet synovial cyst as a possible cause of thoracic myelopathy. The SC was removed thoroughly by laminectomy. The patient had an excellent recovery. The etiological and therapeutic aspects are discussed.

Synovial cyst on prevertebral space of C1-2 joint is rare but may be associated hemorrhagic event. We describe a case of a 72-year-old woman who presented with sudden severe headache in her left occipital area with dyspnea. She had rheumatoid arthritis for 14-years. Large hemorrhagic cystic mass was seen around prevertebral space of the atlantoaxial joint on the left side on cervical MRI (magnetic resonance image) and it obstructed the nasopharyngeal cavity. Aspiration of the cystic lesion was performed via transoral approach, followed by posterior occipito-cervical fusion. The specimen was xanthochromic, suggesting old hemorrhage. The patient was tolerable on her postoperative course and showed good respiration and relieved headache. We suggest that repeated microtrauma due to atalantoaxial subluxation associated with rheumatoid arthritis as a main cause of hemorrhagic event on the cyst.

OBJECTIVE
This retrospective study of 13 patients who underwent surgical treatment for symptomatic lumbar synovial cyst was performed to evaluate the clinical findings and pathogenesis of lumbar synovial cyst.
METHODS
The clinical characteristics of the patients were investigated by reviewing the hospital records, preoperative radiological images, and operation records. By observing preoperative CT scans Facet degeneration grade at the lesion and opposite side of pathologic level and adjacent levels were assessed and compared.
RESULTS
There were 5 males and 8 females (average 65.8 year-old). Six patients presented with low back pain and leg pain, and 7 patients presented only leg pain. Most common pathologic level was L4-5. All patients underwent the cyst resection with/without decompressive laminectomy or discectomy. The additional instrumentation was not performed in all patients. No complications or recurrence was observed during average 34.5 months follow-up. There was no significant difference of facet degeneration grade between the lesion side of pathologic level and opposite side of same level or lower adjacent level.
CONCLUSION
In the present study, all patients showed clinical improvement by the simple surgery without any instrumentation. No significant correlation between the occurrence of synovial cyst and the degeneration grade of facet joint was revealed.

We report a rare case that showed aggravation of neurological symptoms due to expansion of the synovial cyst at C7/T1 after several month of trauma and present the consideration of proper prevention and management. A 64-year old male was admitted by right arm weakness(GIV+) after a pedestrian traffic accident. According to computed tomography(CT) scan and electromyography(EMG), a brachial plexus injury was diagnosed and he was treated conservatively. After 7 months, he was re-admitted by the left side weakness(GIV-) with severe pain and magnetic resonance image(MRI) revealed the expanded cystic lesion at C7/T1 level which compressed the cord from left side. After administration of steroid, surgical resection was performed via posterior approach and partial laminectomy. The dural expansion was observed after total removal of cyst which was diagnosed as a pathologist and symptoms were completely recovered. Because of its possibility of expansion of the synovial cyst and critical myelopathic symptoms can be induced in cervical spine, immobilization should be in consideration for acute period of post-trauma, especially, in old-aged patients with degenerative facet joints. And surgical procedure should not be delayed if symptoms developed.

There have been many reports about lumbar synovial cysts, discussing surgical or interventional treatment methods. However, there have been few reports about spontaneous regression of the cyst. We report a case of a lumbar synovial cyst that regressed spontaneously. To the best of our knowledge, this is the first report of a spontaneously regressed lumbar synovial cyst in Korea. The patient was a 59-year-old man who presented with a 6-month history of insidious pain onset in the lower back and right buttock. Upon MRI of the lumbar spine, grade 1 spondylolisthesis was revealed at the L4-5 level. An about 6mm-sized extradural cyst was detected at the medial side of the right L4-5 facet joint. His symptoms were disappeared upon facet block with 0.5% bupivacaine bilaterally at the L4-5 level. Six years later, he revisited our clinic due to low back pain and left leg radiating pain. Upon lumbar MRI, aggravated L4-5 spondylolisthesis with left-side disc protrusion was observed. However, we could not find the synovial cyst around the right L4-5 facet joint that had existed 6 years prior. The patient was treated conservatively because he refused surgery.

A spinal synovial cyst is an uncommon lesion associated with degenerative spinal arthropathy. It can cause radicular symptoms and may masquerade clinically as other, more common entities. A 50-year-old female presented with an intraspinal synovial cyst at the L5-S1 level manifesting as lower back pain and right leg pain. The patient's clinical symptoms improved after surgical management. Intraspinal synovial cyst should be considered in the differential diagnosis of the patients presenting with acute radicular symptoms.
