Clinical Analysis of Degenerative Disc Disease Treated with Total Disc Replacement

Abstract
Objective
To assess outcome and preservation of angular movement after total disc replacement(TDR) in lumbar degenera- tive disc disease.

Methods
From January, 2002 to December, 2003, 30 patients with degenerative disc disease underwent total disc repla- cement. All patients had low back pain with/without leg pain not respond to conservative treatment for 6 months. We used 12 cases of SB Charite III, 18 cases of Prodisc and did 13 cases at L4/5, 17 cases at L5S1 that were single level in all cases. Mean age was 48(32節ž68) years old. Mean follow-up period was 15 months. We measured angular movement operated level and evaluated outcome with Visual Analogue Scale(VAS) and Oswestry Disability Index(ODI) pre and postoperatively.

Results
In mean follow-up of 15 months VAS score was improved 7 to 3 and ODI was reduced 64% to 18%. Mean angular movement at operated level was 9.7 degrees at L4/5, 8.7 degrees at L5/S1 and 8.6 degrees in Prodisc, 10.3 degrees in SB Charite III. At adjacent level above operated site angular movement showed no difference statistically compared to preoperative movement regardless of level and type of artificial disc.

Conclusion
Total disc replacement can be effective treatment in degenerative disc disease if well-indicated. Prevention of adjacent level degeneration should be followed up for a long time.
