Comparative Analysis Between Orion Locking and ABC Dynamic Plate-Screw Systems in Degenerative Cervical Disc Disease

Abstract
Objective
The purpose of study was to compare the effectiveness of ABC dynamic and Orion screw-plate system after anterior cervical discectomy fusion(ACDF) or anterior cervical corpectomy fusion(ACCF) in the treatment of degenerative cervical disease. Orion system uses a fixed angle screws and its plate accommodates locking screws. In contrast to Orion system, ABC system(Aesculap, San Francisco, CA) is composed of a variable trajectory screw, slotted plates allowing vertical migration of the fixed screw.

Materials and Methods
Sixty-three patients were reviewed and the procedure was the same for both groups(Smith- Robinson with autograft). Group I were 31 patients treated with Orion locking screw-plate. Group II were 32 patients with ABC dynamic screw-plate. Radiological evaluation including bony fusion, graft subsidence, and instrument-related complications and clinical outcome based on Odom`s criteria were performed.

Results
There were no significant differences in clinical outcome between two groups(Group I: excellent; 36%, good; 64% VS Group II: excellent; 40%, good; 60%). The overall fusion rate was higher in Group II(97%) than Group I(90%). The mean height and rate of subsidence were lower in Group I(0.99±0.96mm, 11.1±10.5%) than Group II(1.87±1.33mm, 19.9±10.7%). there was no instruments-related complication in Group II while Group I was revealed 3 cases of instruments-related complications

Conclusion
Although both anterior cervical plate systems demonstrated effective instrumentation after ACDF or ACCF in degenerative cervical disc disease, ABC system seems to be more advantageous with a higher bony fusion and low incidence of instrument-related complications except higher rate of subsidence than Orion system.
