Abstract |
Spinal extradural arachnoid cysts are rare expanding lesions in the spinal canal. They usually present with progressive signs and symptoms caused by spinal cord compression if they enlarge. This report describes a patient with an extradural arachnoid cyst in the thoracic spine, and the possible pathogenesis, radiologic features, and the implications on surgical therapy are discussed.
The patient presented with progressive back pain radiating to both legs along the posterior thigh. Magnetic resonance imaging (MRI) revealed a thoracic extradural arachnoid cyst. Thoracic laminectomy was performed and a large, thin-walled cyst containing CSF was encountered. The walls of the cyst were easily dissected from the dura. The cyst was found to arise from a small neck protruding through a defect in the right lateral aspect of the dura. The neck was obliterated and the dura was repaired.
Postoperatively, the patient was neurologically normal without any deficits. MRI was helpful in diagnosing the lesion. Surgical removal is treatment of choice in patients with symptomatic extradural arachnoid cyst. |
