Patients with lumbosacral extraforaminal disc herniations can present with atypical symptoms, which sometimes causes the difficulty in making a correct diagnosis. We report two patients with lumbosacral extraforaminal disc herniations, who presented with atypical clinical symptoms; one with anterolateral thigh pain and the other with anterior ankle pain. Both patients were not properly diagnosed and treated initially because of their atypical symptoms. Using selective nerve root block based on computed tomography and magnetic resonance imaging results, the patients were treated successfully by paramedian tangential approach.
