Multiple Spinal Ossifications in a Patient with Scleromyxedema |

Abstract |
The authors describe a case of multiple spinal ossifications in a 56-year-old man with scleromyxedema. The patient presented with neck pain and both shoulder pain secondary to compressing lesion to the spinal cord at C5-6 and C6-7 caused by ossification of the posterior longitudinal ligament(OPLL). He also presented with a skin thickening between the posterior superior cervical spine and the interscapular region. Surgical decompression and skin biopsy were performed. Histopathological examination showed marked mucin deposits in the upper and middle dermis and a diagnosis of scleromyxedema was made. A whole spine magnetic resonance(MR) imaging revealed ossification of the ligamentum flavum(OLF) at L1-2 and T10-11. One year postopera- tively the patient was free of neck pain and shoulder pain and without any changes in his neurological status. Skin lesion revealed no further aggravation.
This first report of multiple spinal ossifications in a patient with scleromyxedema illustrates that the potential presence of multiple spinal ossifications must be considered when planning a treatment on a patient with extensive thickening of skin and papules whose pathological examination demonstrates scleromyxedema. |
